Preheat the oven to 285 F (140 C) and butter the ramekins.
Heat the milk without boiling it.
In a bowl, whisk the eggs, sugar, and vanilla.
Still whisking, pour in the hot milk.
Place the buttered ramekins in a baking dish to make a water bath.
Strain the cream through a sieve over the ramekins to fill them, then sprinkle nutmeg over them.
Pour boiling water into the baking dish, up to half the height of the ramekins.
Bake for 1h30. The cream should be just set.
Remove the baking dish from the oven and let it cool slightly before serving.
